• Integrates home care devices like glucometers to capture remote patient reported data, thereby providing continuum of care visibility to care teams

• Provides real-time data analytics, population health oriented dashboards to identify alert conditions, pre-emptively take action and route tasks to available staff so to achieve goals with less resources

MSK acts as a digitalized structured, clinically oriented content and metadata repository of over 2000+ conditions, 300+ symptoms, and tens of other clinical objects each deeply nested with multiple attributes.
